മനുഷ്യരിലും മറ്റു ചില മൃഗങ്ങളിലും ഉണ്ടാകാവുന്ന ഒരു പകർച്ചവ്യാധി വൈറൽ രോഗമാണ് കുരങ്ങുവസൂരി അല്ലെങ്കിൽ വാനരവസൂരി എന്നറിയപ്പെടുന്ന മങ്കിപോക്സ്. [1] പനി, വീർത്ത ലിംഫ് നോഡുകൾ, കുമിളകൾ രൂപപ്പെടുകയും പിന്നീട് പുറംതോട് രൂപപ്പെടുകയും ചെയ്യുന്ന ചുണങ്ങ് എന്നിവയാണ് ലക്ഷണങ്ങൾ. [1] അണുബാധയുണ്ടായി 5 മുതൽ 21 ദിവസംത്തിനകം രോഗലക്ഷണങ്ങൾ ആപ്രത്യക്ഷപ്പെടുന്നു.[3] [5] രോഗലക്ഷണങ്ങളുടെ ദൈർഘ്യം സാധാരണയായി 2 മുതൽ 4 ആഴ്ച വരെയാണ്. [5] നേരിയ ലക്ഷണങ്ങൾ ഉണ്ടാവുകയോ രോഗലക്ഷണങ്ങളൊന്നും അറിയാതെ രോഗബാധയുണ്ടാവുകയോ ചെയ്യാം. [3] [17] പനിയും മറ്റ് ലക്ഷണങ്ങളും എല്ലാ പകർച്ചയിലും പൊതുവായി കണ്ടെത്തിയിട്ടില്ല. ഓർത്തോപോക്സ് വൈറസ് ജനുസ്സിലെ സൂനോട്ടിക് വൈറസായ മങ്കിപോക്സ് വൈറസാണ് ഈ രോഗത്തിന് കാരണമാകുന്നത്. വസൂരിയുടെ കാരണക്കാരനായ വേരിയോള വൈറസും ഈ ജനുസ്സിൽ പെട്ടതാണ്. [2] മനുഷ്യരിലെ രണ്ട് തരങ്ങളിൽ, പശ്ചിമാഫ്രിക്കൻ തരം മധ്യ ആഫ്രിക്കൻ (കോംഗോ ബേസിൻ) തരത്തേക്കാൾ കഠിനമായ രോഗത്തിന് കാരണമാകുന്നു. [20] രോഗം ബാധിച്ച മൃഗങ്ങളിൽ നിന്നോ, രോഗം ബാധിച്ച മാംസം കൈകാര്യം ചെയ്യുന്നതിലൂടെയോ മൃഗങ്ങളുടെ കടികളിലൂടെയോ പോറലുകൾ വഴിയോ ഇത് പകരാം. [21] രോഗബാധിതമായ ശരീര സ്രവങ്ങൾ, മലിനമായ വസ്തുക്കൾ, വായുവിലൂടെയുള്ള സമ്പർക്കം എന്നിവയിലൂടെ മനുഷ്യരിൽ നിന്ന് മനുഷ്യരിലേക്ക് പകരാം . [1] [21] രോഗലക്ഷണങ്ങളുടെ തുടക്കം മുതൽ രോഗം പൂർണ്ണമായി ഭേദമാകുന്നതുവരെ, ആളുകൾക്ക് വൈറസ് പകരാൻ കഴിയും. [20] വൈറസിന്റെ ഡിഎൻഎ പരിശോധിച്ച് രോഗനിർണയം നടത്താം. [8]

അറിയപ്പെടുന്ന ചികിത്സയില്ല. [22] 1988-ൽ നടത്തിയ ഒരു പഠനത്തിൽ, വസൂരി വാക്സിൻ അടുത്ത സമ്പർക്കത്തിൽ അണുബാധ തടയുന്നതിനും രോഗത്തിന്റെ തീവ്രത കുറയ്ക്കുന്നതിനും ഏകദേശം 85% സംരക്ഷണമാണെന്ന് കണ്ടെത്തി. [23] പരിഷ്കരിച്ച വാക്സിനിയ അങ്കാറയെ അടിസ്ഥാനമാക്കിയുള്ള ഒരു പുതിയ വസൂരി വാക്സിൻ അംഗീകരിച്ചു, എന്നാൽ ലഭ്യത പരിമിതമാണ്. [3] വ്യക്തിശുചിത്വം പാലിക്കുക, രോഗികളും മറ്റ് മൃഗങ്ങളുമായുള്ള സമ്പർക്കം ഒഴിവാക്കുക എന്നിവയാണ് രോഗപ്പകർച്ച തടയാനുള്ള മറ്റു മാർഗ്ഗങ്ങൾ. [24] ആൻറിവൈറൽ മരുന്നുകൾ, സിഡോഫോവിർ, ടെക്കോവിരിമാറ്റ്, വാക്സിനിയ ഇമ്യൂൺ ഗ്ലോബുലിൻ, വസൂരി വാക്സിൻ എന്നിവ രോഗപ്പകർച്ച നിയന്ത്രിക്കാൻ ഉപയോഗിക്കാം. [12] [13] രോഗം സാധാരണയായി സൗമ്യമാണ്. രോഗബാധിതരിൽ ഭൂരിഭാഗവും ചികിത്സ കൂടാതെ ഏതാനും ആഴ്ചകൾക്കുള്ളിൽ സുഖം പ്രാപിക്കും. [13] മരണസാധ്യതയെക്കുറിച്ചുള്ള ഏകദേശ കണക്കുകൾ 10% വരെയാണ്. എന്നിരുന്നാലും 2017 മുതൽ കുരങ്ങുപനിയുടെ അനന്തരഫലമായി വളരെ കുറച്ച് മരണങ്ങൾ മാത്രമേ രേഖപ്പെടുത്തിയിട്ടുള്ളൂ [25] .

1958-ൽ ഡെൻമാർക്കിലെ കോപ്പൻഹേഗനിലെ ലബോറട്ടറി കുരങ്ങുകൾക്കിടയിലാണ് മങ്കിപോക്സ് ആദ്യമായി ഒരു പ്രത്യേക രോഗമായി തിരിച്ചറിഞ്ഞത്. [26] നിരവധി ഇനം മൃഗങ്ങൾ വൈറസിന്റെ സ്വാഭാവിക വാഹകരായി പ്രവർത്തിക്കുന്നുണ്ടെന്ന് സംശയിക്കുന്നു. [27] ഒരു കാലത്ത് മനുഷ്യരിൽ ഇത് അസാധാരണമാണെന്ന് കരുതിയിരുന്നെങ്കിലും, 1980 മുതൽ കേസുകൾ ഗണ്യമായി വർദ്ധിച്ചു, [28] [29][9] [14] 1970 ൽ ഡെമോക്രാറ്റിക് റിപ്പബ്ലിക് ഓഫ് കോംഗോയിൽ (ഡിആർസി) മനുഷ്യരിൽ ആദ്യമായി രോഗം കണ്ടെത്തി. [30] മധ്യ ആഫ്രിക്കയിലും പശ്ചിമാഫ്രിക്കയിലും ഇടയ്ക്കിടെ കേസുകൾ ഉണ്ടായിട്ടുണ്ട്.[29] 2022-ലെ കുരങ്ങുപനി പൊട്ടിപ്പുറപ്പെടുന്നത് ആഫ്രിക്കയ്ക്ക് പുറത്ത് വ്യാപകമായ സമൂഹവ്യാപനത്തിന്റെ ആദ്യ സംഭവത്തെ പ്രതിനിധീകരിക്കുന്നു, ഇത് യുണൈറ്റഡ് കിംഗ്ഡത്തിൽ 2022 മെയ് മാസത്തിൽ ആദ്യം തിരിച്ചറിഞ്ഞു, യൂറോപ്പ്, വടക്കേ അമേരിക്ക, തെക്കേ അമേരിക്ക, ഏഷ്യ, ആഫ്രിക്ക, ഓസ്ട്രേലിയ. തുടങ്ങിയ 20 രാജ്യങ്ങളിൽ കേസുകൾ സ്ഥിരീകരിച്ചു. തലവേദന, പേശിവേദന, പനി, ക്ഷീണം എന്നിവയാണ് പ്രാരംഭ ലക്ഷണങ്ങൾ . [3] [36] ഇത് തുടക്കത്തിൽ ഇൻഫ്ലുവൻസ പോലെ പ്രത്യക്ഷപ്പെടാം. [37] ഈ രോഗം ചിക്കൻപോക്സ്, അഞ്ചാംപനി, വസൂരി എന്നിവയോട് സാമ്യമുള്ളതാണ്, പക്ഷേ വീർത്ത ഗ്രന്ഥികളുടെ സാന്നിധ്യത്താൽ ഇത് വേർതിരിച്ചിരിക്കുന്നു. [17] 2022-ലെ കുരങ്ങുപനി പൊട്ടിപ്പുറപ്പെട്ട പല കേസുകളിലും ജനനേന്ദ്രിയത്തിലും പൊള്ളലുകൾ കാണപ്പെടുകയും പനി, വീർത്ത ലിംഫ് നോഡുകൾ, വിഴുങ്ങുമ്പോൾ വേദന എന്നിവയും ഉണ്ടായിരുന്നു. [1]

രോഗം ബാധിച്ചവരിൽ കാലുകളിലും വായിലും, ജനനേന്ദ്രിയത്തിലും കണ്ണുകളിലും കുമിളകൾ കാണപ്പെട്ടു. [3] അവ ചെറിയ പരന്ന പാടുകളായി ആരംഭിക്കുന്നു, കുമിളകൾ ദ്രവം നിറഞ്ഞ് പിന്നീട് പൊട്ടിത്തെറിക്കുകയും ചുണങ്ങുകയും ചെയ്യുന്നു. [5] [36] കുമിളകൾ ഒന്നുചേർന്ന് ചിലപ്പോൾ വലിയ മുറിവുകൾ ഉണ്ടാകാം. [3]

ശരീരത്തിന്റെ ഓരോ ബാധിത ഭാഗങ്ങളിലും, മുറിവുകൾ ഒരേ ഘട്ടത്തിൽ വികസിക്കുന്നു. [2] ഇത് വസൂരിയുടെ ചുണങ്ങുപോലെ കാണപ്പെടുന്നു. [38] ചുണങ്ങ് സാധാരണയായി പത്ത് ദിവസം നീണ്ടുനിൽക്കും. ദ്വിതീയ അണുബാധകൾ, ന്യുമോണിയ, സെപ്സിസ്, എൻസെഫലൈറ്റിസ്, ഗുരുതരമായ നേത്ര അണുബാധയാണെങ്കിൽ കാഴ്ചശക്തി നഷ്ടപ്പെടൽ എന്നിവ സങ്കീർണതകളിൽ ഉൾപ്പെടുന്നു. [3] ഗർഭാവസ്ഥയിൽ അണുബാധയുണ്ടായാൽ, ചാപിള്ള പ്രസവംമോ ജനന വൈകല്യങ്ങളോ ഉണ്ടാകാം. [40] കുട്ടിക്കാലത്ത് വസൂരി വാക്സിനേഷൻ എടുക്കുന്നവരിൽ ഈ രോഗം കുറവായിരിക്കാം. [13]

മനുഷ്യരിലും മൃഗങ്ങളിലും മങ്കിപോക്സ് ഉണ്ടാകുന്നത് മങ്കിപോക്സ് വൈറസിന്റെ അണുബാധ മൂലമാണ് - ഓർത്തോപോക്‌സ് വൈറസ് ജനുസ്സിലെ, പോക്‌സ്‌വിറിഡേ കുടുംബത്തിലെ ഡബിൾ സ്‌ട്രാൻഡഡ് ഡിഎൻഎ വൈറസ് ആണിത് . [7] മധ്യ, പടിഞ്ഞാറൻ ആഫ്രിക്കയിലെ ഉഷ്ണമേഖലാ മഴക്കാടുകളിൽ പ്രധാനമായും ഈ വൈറസ് കാണപ്പെടുന്നു. മനുഷ്യരിൽ കുരങ്ങുപനിയുടെ മിക്ക കേസുകളും രോഗബാധിതനായ ഒരു മൃഗത്തിൽ നിന്നാണ് ഉണ്ടാകുന്നത്, എന്നിരുന്നാലും പകരുന്ന വഴി അജ്ഞാതമായി തുടരുന്നു. മുറിവുള്ള ചർമ്മം, ശ്വാസനാളം, അല്ലെങ്കിൽ കണ്ണുകൾ, മൂക്ക് അല്ലെങ്കിൽ വായ എന്നിവ വഴി വൈറസ് ശരീരത്തിൽ പ്രവേശിക്കുമെന്ന് കരുതപ്പെടുന്നു. [6] ഒരു മനുഷ്യന് രോഗം ബാധിച്ചാൽ, മറ്റ് മനുഷ്യരിലേക്ക് പകരുന്നത് സാധാരണമാണ്, കുടുംബാംഗങ്ങൾക്കും ആശുപത്രി ജീവനക്കാർക്കും പ്രത്യേകിച്ച് അണുബാധയ്ക്കുള്ള സാധ്യത കൂടുതലാണ്. [6]

മനുഷ്യനിൽ നിന്ന് മനുഷ്യനിലേക്ക് പകരുന്നത് രോഗബാധിതനുമായുള്ള അടുത്ത സമ്പർക്കത്തിലൂടെയാണ് സംഭവിക്കുന്നതെന്ന് കരുതപ്പെടുന്നു. ലൈംഗിക ബന്ധത്തിൽ ഏർപ്പെടുമ്പോൾ രോഗം പകരുന്നതായി സൂചനയുണ്ട്. [41][42]


കുരങ്ങുകൾക്ക് പുറമേ, ഗാംബിയൻ പൗച്ച് എലികളിലും ( Cricetomys gambianus ), ഡോർമിസ് ( Grafiurus spp.), ആഫ്രിക്കൻ അണ്ണാൻ ( Heliosciurus, Funisciurus ) എന്നിവയിലും വൈറസ് കാണപ്പെടുന്നു. ഈ മൃഗങ്ങളെ ഭക്ഷണമായി ഉപയോഗിക്കുന്നത് മനുഷ്യരിലേക്ക് പകരുന്നതിനുള്ള ഒരു പ്രധാന ഉറവിടമായിരിക്കാം. [43]

കുരങ്ങുപനിക്ക് പ്രത്യേക റിസർവോയർ കണ്ടെത്തിയിട്ടില്ല. പേരിന് വിരുദ്ധമായി കുരങ്ങുകൾ ഒരു പ്രധാന റിസർവോയറല്ല. മുകളിൽ ലിസ്റ്റുചെയ്തിരിക്കുന്നതുപോലുള്ള ആഫ്രിക്കൻ എലികൾ യഥാർത്ഥ വാഹകരായി വർത്തിക്കുന്നു എന്ന് വിശ്വസിക്കപ്പെടുന്നു. [3][44][45]

വസൂരി വാക്സിൻ എടുത്തവരിൽ കുരങ്ങ്പോക്സ് സാധ്യത കുറയ്ക്കുന്നതായി റിപ്പോർട്ട് ചെയ്യപ്പെട്ടിട്ടുണ്ട്. പോക്‌സ് വൈറസുകൾക്കുള്ള പ്രതിരോധശേഷി കുറയുന്നതാണ് കുരങ്ങുപനിയുടെ വ്യാപനത്തിന് കാരണമാകുന്നത്. 1980-ന് മുമ്പ് വൻതോതിൽ വസൂരി വാക്സിനേഷനുകൾ നിർത്തലാക്കിയപ്പോൾ വാക്സിനേഷൻ എടുത്തവരിൽ ക്രോസ്-പ്രൊട്ടക്റ്റീവ് പ്രതിരോധശേഷി ക്ഷയിച്ചതും വാക്സിനേഷൻ ചെയ്യാത്ത വ്യക്തികളുടെ ക്രമേണ വർദ്ധിച്ചുവരുന്ന അനുപാതവുമാണ് ഇതിന് കാരണം. യുണൈറ്റഡ് സ്റ്റേറ്റ്സ് സെന്റർസ് ഫോർ ഡിസീസ് കൺട്രോൾ ആൻഡ് പ്രിവൻഷൻ (സിഡിസി) കുരങ്ങുപനി പടർന്നുപിടിക്കുന്നതിനെക്കുറിച്ച് അന്വേഷിക്കുന്നവരും രോഗബാധിതരായ വ്യക്തികളെയോ മൃഗങ്ങളെയോ പരിപാലിക്കുന്നതിൽ ഏർപ്പെട്ടിരിക്കുന്നവരും കുരങ്ങുപനിക്കെതിരെ പരിരക്ഷിക്കുന്നതിന് വസൂരി വാക്സിനേഷൻ സ്വീകരിക്കണമെന്ന് ശുപാർശ ചെയ്യുന്നു. കുരങ്ങുപനി സ്ഥിരീകരിച്ച വ്യക്തികളുമായോ മൃഗങ്ങളുമായോ അടുത്തോ അടുത്തോ സമ്പർക്കം പുലർത്തിയിട്ടുള്ള വ്യക്തികൾക്കും വാക്സിനേഷൻ നൽകണം. രോഗബാധിതനായ വ്യക്തിയെ പരിചരിക്കുന്നതിന് മുമ്പ് ആരോഗ്യ പരിരക്ഷാ ദാതാക്കൾ പൂർണ്ണമായ വ്യക്തിഗത സംരക്ഷണ ഉപകരണങ്ങൾ ( പിപിഇ ) ഉപയോഗിക്കണമെന്ന് സിഡിസി ശുപാർശ ചെയ്യുന്നു. ഇതിൽ ഒരു ഗൗൺ, മുഖംമൂടി, കണ്ണട, ഫിൽട്ടറിംഗ് ഡിസ്പോസിബിൾ റെസ്പിറേറ്റർ ( N95 പോലുള്ളവ) എന്നിവ ഉൾപ്പെടുന്നു. രോഗബാധിതനായ ഒരു വ്യക്തിയെ ഐസൊലേറ്റ് ചെയ്യണം.
