ജാൻ തോമസ് ഫോർമാൻ (ജീവിതകാലം : 18 ഫെബ്രുവരി 1932 – 14 ഏപ്രിൽ 2018) മിലോസ് ഫോർമാൻ എന്നു പൊതുവായി അറിയപ്പെട്ടിരുന്ന അദ്ദേഹം ഒരു ചെക്ക്- അമേരിക്കൻ ചലച്ചിത്ര സംവിധായകൻ, തിരക്കഥാകൃത്ത്, അഭിനേതാവ്, പ്രൊഫസർ എന്നീ നിലകളിൽ പ്രശസ്തനായിരുന്നു. അദ്ദേഹം 1968 വരെ പ്രാഥമികമായി മുൻ ചെക്കോസ്ലോവാക്യയിൽ ജീവിക്കുകയും ജോലി ചെയ്യുകയും ചെയ്തിരുന്നു.
